Our organization is seeking a dedicated and compassionate Clinical Aged Care Professional to join our team. In this role, you will be responsible for conducting thorough assessments of older adults to identify their needs and connecting them with essential services that support independent living.
The ideal candidate will possess a strong background in health-related disciplines, excellent communication skills, and the ability to work collaboratively with families and carers. You will also have a current unrestricted General Registration with AHPRA or relevant professional association, as well as proficiency in client management systems.
Key Responsibilities:
* Aged Care Assessments: Conduct thorough assessments to identify the needs of older adults.
* Service Referrals: Facilitate referrals to essential services that support independent living.
* Promoting Functionality: Actively promote the retention and maximization of function and capability in daily living.

What We Look For:
* At least 1 year of experience in aged care, including client assessments and/or service delivery.
* Tertiary education in a health-related discipline.
* Current unrestricted General Registration with AHPRA or relevant professional association.
* Proficiency in Word, Excel, Outlook, and client management systems.
