We are now seeking an experienced and values-driven Program Manager – Home Care to lead our Support at Home Package services and strengthen the quality, growth and impact of this vital program.

The Opportunity
As Program Manager – Home Care, you will lead the operational delivery, development and clinical oversight of our Home Care Package program. Reporting to the General Manager Home Services, you will drive service excellence, ensure compliance with Aged Care Quality Standards, and provide supportive leadership to Care Managers and support staff.
Your role will be central in shaping how we deliver safe, high-quality, person-centred services that enable older people to remain independent in their homes.

Key Responsibilities
Program Leadership & Growth
* Lead and grow the Home Care Package program, identifying opportunities to expand services and strengthen client outcomes.
* Develop and implement growth and marketing strategies tailored to Home Care.
* Build a deep understanding of our value proposition and promote service uptake.
* Manage intake processes and ensure individual plans are evidence-based and aligned to clinical and lifestyle needs.
Quality, Clinical Governance & Compliance
* Oversee compliance with Aged Care Quality Standards, ISO frameworks and WHS requirements.
* Provide clinical governance and high-level advice to ensure safe, best-practice service delivery.
* Identify and manage risk, including clinical risk, across the program.
* Monitor performance through reporting, data analysis and quality reviews.
Team Leadership & Workforce Support
* Provide strong leadership to Care Managers and support staff.
* Lead recruitment, onboarding, supervision, performance management and staff development.
* Promote a positive staff culture focused on professionalism, wellbeing and continuous improvement.
* Ensure staff are supported to meet skill, compliance and service delivery expectations.
Stakeholder & Relationship Management
* Maintain strong relationships with clients, families, referrers and community partners.
* Collaborate with internal teams and external stakeholders to ensure seamless service delivery.
* Resolve service issues professionally and in a timely manner.
* Represent YellowBridge QLD at sector meetings, forums and community events.
Operational & Administrative Responsibilities
* Deliver accurate reporting, budgeting and program performance monitoring.
* Oversee efficient rostering and service coordination.
* Undertake professional development and contribute to broader organisational initiatives.

What You'll Bring
* Diploma or higher qualification in Community Services or a related field.
* Clinical care experience (clinical qualification desirable but not essential).
* Demonstrated experience managing aged care or community care programs, ideally Support at Home (previously Home Care) Packages.
* Strong understanding of compliance requirements, Aged Care Quality Standards and program reporting.
* Experience in business development and service growth.
* Excellent interpersonal, leadership and stakeholder engagement skills.
* High-level organisational, analytical and decision-making abilities.
