Join our passionate Transitional Aged Care Program (TACP) team and help older adults regain independence and confidence after hospital care. Here, kindness, teamwork, and exceptional client care are at the heart of everything we do and so is supporting you.
As a Physiotherapist Level 1/2, you'll play a vital role in providing high-quality physiotherapy services across both our community-based (Transpac) and residential (Transitional Care Unit) programs. You'll have the opportunity to rotate into the Woy Woy Transitional Care Unit, expanding your skills with ward-based therapy while making a meaningful impact in your community.

About the Opportunity
You will:
* Deliver evidence-based interventions that promote mobility, function, and independence.
* Collaborate with a multidisciplinary team including nurses, occupational therapists, social workers, and medical staff.
* Provide holistic, goal-oriented care to clients and their families, supporting recovery and wellbeing.
We have both permanent part-time and permanent full-time positions available.

About You
We are looking for someone who has:
* Current registration with AHPRA as a Physiotherapist.
* Strong clinical assessment and treatment skills, ideally with older adults or in rehabilitation.
* Experience working collaboratively within a multidisciplinary team.
* An understanding of aged care needs including falls prevention, pain management, and functional recovery.
* Flexibility to work across all sites within CCLHD.
