Fremantle Hospital Mental Health Service is seeking a dedicated and experienced Senior Social Worker to join our dynamic Transitional Care Team.We are committed to providing high-quality social work services to individuals experiencing mental health and homelessness related issues, their families/significant others from a trauma informed and recovery orientated perspective.
