About the Role
* Act as a liaison between consumers, families and health service staff to ensure effective communication and care.
* Support consumers in accessing relevant services that cater to their needs.
* Plan and implement admission, treatment, case management, referral, discharge and follow-up processes for Aboriginal individuals using mental health services.
Key Requirements
To be successful in this role, you must be of Aboriginal or Torres Strait Islander descent and possess a minimum Certificate III qualification in Aboriginal Primary Health Care or a related field. Strong knowledge of social and emotional wellbeing and effective written and verbal communication skills are also essential.
A key aspect of this position is supporting consumers and their families through hospitalisation, understanding the social, psychological and financial implications it may have. You will need to work collaboratively with healthcare professionals to deliver high-quality care.
