Role Overview:
Our Wellbeing Practitioner plays a vital role in building meaningful relationships with clients and connecting them with local resources, health services, and other support networks.
Main Responsibilities:
* Working closely with veterans and their families within homelessness and financial hardship initiatives
* Conducting thorough assessments and developing case management plans that focus on outcomes-based approaches to veteran wellbeing
* Preparing documentation to secure support from relevant organizations
* Visiting clients in their homes or hospitals to provide emotional support and connect them with additional resources
Key Requirements:
* Completion of a degree qualification in Humanities, Social Welfare, Health Sciences, or Social Science
* Significant knowledge and experience in Strength-based Case Management
* Aptitude for and interest in pursuing ATDP or MILADW001 in Welfare
* Demonstrated ability to manage client workloads effectively in sensitive environments
* Excellent communication and interpersonal skills to build rapport and trust
