We're a small but mighty social work and support coordination team on the Sunshine Coast. We work alongside people with complex needs — bringing heart, integrity, and smarts to a space that can often feel confusing or disempowering. About You
You might be a social worker, educator, or have experience in mental health, disability, child safety, hospital discharge, or other case management roles. You're confident in your skills but still open to learning.
We’re looking for someone who:
Can handle complexity– you’re good at holding space for things that don’t always fit neatly in boxes
Is collaborative and relational– you work well with people and value communication that’s real and respectful
Can advocate in tough systems– you hold your ground with grace, speak up when it matters, and act with integrity
Has a sense of humour– you take the work seriously, but not yourself
Has facilitation and stakeholder management skills– you can bring people together, whether in meetings, planning sessions, or tough conversations
Is values-driven and curious– you're always learning, reflecting, and trying to do better
The Role – Support Coordinator
This isn’t just about ticking NDIS boxes. It’s about being a strong, steady presence for people navigating complex systems. You’ll:
Walk alongside clients as they implement their NDIS plans
Help connect the dots between services, goals, and lived experience
Navigate systems and advocate for what’s fair and needed
Facilitate conversations and planning sessions
Bring clarity and heart to report writing and documentation
Work autonomously, but with the full support of our close-knit team
Experience in Level 3 / Specialist Support Coordination is highly desirable but not essential.
You’ll Need:
A degree in social work, education, human services, psychology, or a related field
A strong understanding of person-led, rights-based practice
Experience with NDIS or similar complex systems (or a fast learner)
Excellent relational and communication skills
Confidence with admin, email, and navigating systems
Driver’s license and reliable transport
Blue Card and Yellow Card (or willingness to obtain)
