About the Role:
* Support older Australians by managing a caseload of clients, developing person-centred care plans, and providing holistic support tailored to their medical, emotional, and social needs.

Key Responsibilities
* Conduct comprehensive assessments of clients' health and social care needs
* Develop, implement, and review individualised care plans aligned with best practices and client goals
* Coordinate services with internal and external providers including GPs, allied health professionals, and community services
* Advocate for client wellbeing, rights, and access to resources and supports
* Provide clinical guidance, emotional support, and health education to clients and their families
* Maintain accurate and up-to-date documentation in accordance with regulatory standards
* Monitor outcomes and adjust care plans to reflect changing client needs
About You
* AHPRA registration as a Registered Nurse (essential)
* Experience in case management, preferably within aged care or community settings
* Strong interpersonal and communication skills with a compassionate, person-centred approach
* Excellent time management and organisational skills
* Understanding of the Aged Care Quality Standards and community care frameworks
* Ability to work independently and as part of a multidisciplinary team
* Current driver's licence and access to a reliable vehicle (some travel required)
