About us:
The Rapid Access Chronic Disease clinics is a dynamic workplace which treats adult patients with a range of chronic conditions – wound care, respiratory and diabetes. These services are provided at a range of locations across Rockhampton and via home visits. The service is staffed by a team of Nurse Practitioners, Clinical Nurses, a part-time Respiratory Physician and the Aboriginal and Torres Strait Isander Health Practitioner.
About the role:
The Aboriginal and Torres Strait Islander Health Practitioner will provide a range of clinical primary health care services to Aboriginal and Torres Strait Islander clients including delivering specific health care programs, assessing, and treating clients, maintaining health care systems and providing advice and support whilst under clinical supervision of the Nurse Practitioner to contribute to better health outcomes for Aboriginal and Torres Strait Islander people.
The position will work across the chronic disease speciality, allowing flexibility wherever clinical and cultural needs are identified. Duties will include hands‐on clinical care such as wound management and respiratory assessments, along with general chronic disease support. Training, supervision and ongoing development will be provided to ensure the clinician is supported to maintain required skills.

* As part of a multi-disciplinary team, provide a culturally responsive primary healthcare service to Aboriginal and Torres Strait Islander people and their families, within a defined scope of practice and healthcare plans.
* Support Aboriginal and Torres Strait Islander people to navigate the health system, ensuring they have continuity of care across the healthcare continuum.
* In partnership with other health professionals provide health education and promotion programs in a community.
* Engage with community members to strengthen community involvement in health service delivery.
About you:
* Hold current registration as an Aboriginal and Torres Strait Islander Health Practitioner as recognised by the Aboriginal and Torres Strait Islander Health Practitioner Board of Australia (ATSIHPBA) is mandatory for this position.
* Successful completion of the Certificate IV or Diploma in Aboriginal and Torres Strait Islander Primary Health or equivalent is mandatory for this position.
* Applicants who do not hold the mandatory qualification requirements, but believe they have the knowledge, skills and abilities to perform the role as outlined, and will commit to completing the qualification(s) within an agreed timeframe if appointed, are encouraged to apply for this vacancy.
* Under Section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.
What we offer:
Total remuneration value of up to $121,350 p.a. is comprised of:
* An hourly rate between $39.00 - $53.64 p.h.
* 12.75% employer superannuation contribution
