What You'll Be Doing
IMHAOD Declaration
Each person's unique journey of recovery will be supported by mental health drug and alcohol services in a way that fosters hope, purpose and resilience.
IMHAOD Statement of Intention
The intention of the MNCLHD Integrated Mental Health and Alcohol and Other Drugs Services (IMHAOD) is to provide recovery-oriented, trauma informed services that are guided by evidence-based practices and collaboration.
The Transition and Physical Health Coordinator is primarily responsible for two primary functions.
* Within the wider multidisciplinary team to coordinate, negotiate, and manage discharge planning needs of consumers to facilitate a smooth transition from hospital to the Community Mental Health Service and coordinate 7-day post discharge follow up clinics
* Coordinate physical and metabolic care pathways ensuring the physical health of consumers is monitored and interventions are put in place.
About You
Do you have Qualifications eligible for membership of the Australian Association of Social Workers; or qualifications recognised for registration with the Occupational Therapy Board of Australia. Registered Nurse - with the Australian Health Practitioner Regulation Agency (AHPRA), Relevant post registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualifications or such other qualifications or relevant equivalent experience; or qualifications eligible for appointment as a Counsellor or Welfare Officer and evidence of clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care.
