ACCORD CARE is trusted in the provision of person-centered disability support services to NDIS participants. We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation, and Community Nursing.
About the role
We are looking for team members in our supported independent living arrangements in Glass House Mountains to provide 24/7 support in the home and out in the community.
The support worker's role and responsibilities will include:
* Supporting people with personal care and day-to-day activities in the home and out in the community
* Providing positive behaviour support to our clients
* Completing appropriate documentation through shift notes, incident reporting and data collection
* Contribute to the team's overall objective of providing the best outcomes for our clients
* Work with your team leader to implement and review plans around daily service provision
* Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
* Above all, be responsive, reliable and accountable to our clients and your team

Qualification/Experience:
* Minimum Certificate III in Individual Support/Aged Care
* (or the ability to acquire an appropriate qualification)
* Proven experience supporting individuals with complex or high behaviours, including aggression and property damage.
* Mental Health Experience
* Strong behaviour management and de-escalation skills.
* Problem-Solving Skills
* Verbal/Written Communication and Relationship Building Skills
