Superior Home Health Services is looking for a caring, confident, and dependable Home and Community Registered Nurse Care Partner to support our Gympie, Noosa and Hinterland clients. You'll be based in the Gympie or Noosa Office and enjoy the variety of travelling to provide exceptional care where it's needed most.
Responsibilities will include:
* Working within a person-centered, enabling environment that respects the rights and needs of each client.
* Manage case management for a variety of aged and disability clients.
* Manage home care funding administrative tasks;
* Assisting clients with personal hygiene, dressing, and domestic duties;
* Assisting clients with daily errands and transport where required;
* Providing support to access social and recreational activities;
* Assisting clients with medications;
* Observe, care and report any changes in the health and wellbeing of the client;
* Undertake skilled nursing tasks where required;
* Responding to emergency situations
Essential Criteria:
* Diploma of Nursing or Bachelor of Nursing - post grad qualifications in relevant areas highly desirable
* Catheter management and continence assessments
* Excellent verbal and written communication skills
* Willingness to learn, develop and take on new challenges
* Current National Police Check
* Current First Aid & CPR certificates
* Current unrestricted driver's license
* NDIS Worker Screening including Blue Card (or equivalent)
* Community experience, 2 year minimum
Preferred Criteria
* Previous Care Coordination/Case Management in Community
* Post Grad qualifications and specialties relevant to community care
* Located Gympie or Sunshine Coast
