Your Key Responsibilities:
* To coordinate and deliver high-quality palliative care services within the community setting;
* To work collaboratively with the multidisciplinary team to develop effective strategies for improving patient outcomes;
* To assess patients' needs and develop personalized plans that cater to their unique requirements;
* To communicate effectively with patients, families, carers, and healthcare professionals while maintaining accurate records.
