You will undertake thorough assessments to formulate and implement appropriate care plans, using high-quality written and verbal communication skills to ensure timely completion of tasks.
Key Responsibilities
* Conduct detailed assessments to develop individualized care plans that meet the complex needs of older individuals.
* Utilize excellent communication skills to effectively interact with multidisciplinary teams, patients, and families.
* Demonstrate in-depth knowledge of My Aged Care and related programs to assess and support both routine and complex presentations.
Required Skills and Qualifications
* Possess relevant tertiary qualifications as per clinical discipline (nursing, occupational therapy, social work, physiotherapy), with full registration or eligibility for membership of the relevant Australian peak professional body.
* Proven ability to utilize Microsoft Office Programs, electronic patient record systems, and e-Learning sites.
* Demonstrated expertise working in aged care settings with a focus on delivering high-quality care.
