 
        
        Role Overview:
This position plays a vital part in identifying the needs of older adults and connecting them with services that enable them to live independently with high quality of life.
Responsibilities:
 * Conduct thorough assessments to identify the needs of older adults.
 * Facilitate referrals to essential services that support independent living.
 * Promote retention and maximization of function and capability in daily living.
 * Work closely with families and carers, recognizing them as key stakeholders.
 * Ensure all assessments are conducted in line with contractual requirements.
Requirements:
 * At least one year of experience in aged care.
 * Tertiary education in a health-related discipline.
 * Current unrestricted registration with AHPRA or relevant professional association.
 * Proficiency in Microsoft Office suite and client management systems.
 * Australian driver's license and comprehensively insured vehicle.
Key Requirements:
 * 1 year aged care experience.
 * Health-related tertiary education.
 * Current General Registration with AHPRA.
 * Microsoft Office proficiency.
 * Australian driver's license.
