Job Title: Disability Support Associate
To provide clients with high-quality support, promoting increased independence, function, quality of life, and community participation in line with their individual goals and aspirations.
Key Responsibilities:
* Assist participants to achieve their identified objectives through personalized strategies.
* Develop and implement tailored programs for each individual client to ensure effective outcomes.
* Maintain accurate records of client progress and adapt interventions as necessary.
* Facilitate client engagement and empowerment through supportive relationships.
* Identify potential risks and mitigate them in accordance with established policies and procedures.
* Cultivate effective communication with clients, their families, and management team members.
* Ensure a safe and clean environment by adhering to daily task lists.
* Notify management promptly of any incidents or issues that may arise.
* Participate in scheduled training and meetings to enhance professional development.
* Support clients in managing their medications according to organizational guidelines.
* Pursue routine administrative duties, including filing, recording, checking, and balancing accounts, invoices, orders, and store requisitions.
* Provide personal care assistance to clients, including but not limited to assistance with continence aids, showering, toileting, and grooming.
* Aid clients in maintaining their homes, encompassing all aspects of domestic duties.
Requirements:
* Hold a Working with Vulnerable People Certificate.
* Possess a current National Police Certificate.
* Hold a valid Drivers' Licence.
* Have a current First Aid certificate.
* Demonstrate knowledge of the Tasmanian Disability Services Act 2012 and the National Disability Insurance Scheme Act 2013.
* Complete the NDIS Core Module.
Work Environment:
The role will involve working within Supported Independent Living settings, at client residences, and in community contexts as required.
