Job Summary

About the Role
* You will be responsible for managing intake by triaging referrals, responding to phone enquiries, registering referrals, and ensuring timely direction of calls and information.
* You will maintain a clinical caseload, conduct assessments, assist with care planning, provide health coaching services link patients effectively across healthcare sectors.
Requirements
* Curent registration as an Enrolled Nurse with the Nursing Midwifery Board Australia (NMBA).
* Able work within your scope practice under supervision whilst prioritizing patient safety person centeredness employable documentation effective communication interpersonal skills respectfulness
