About the Role
As a Telehealth Clinician, you will:
Coordinate care for individuals navigating injury recovery, chronic conditions, post-hospital transitions, or integrated care programs via phone.
Empower patients with the knowledge and tools to manage their recovery holistically.
Liaise with key stakeholders (insurers, healthcare providers, and medical teams) to ensure a timely and sustainable return to health.
Close potential gaps in care by providing health education, reviewing discharge instructions, arranging follow-up services, and addressing future health needs.
Work within a multidisciplinary team to improve the quality of healthcare services.
What We're Looking For
Current unrestricted AHPRA registration as a Registered Nurse (Division 1)
Postgraduate qualifications in Mental Health Nursing (e.g., Graduate Certificate/Diploma in Mental Health Nursing or ACMHN Credential)
Minimum 3 years of post-graduate nursing experience, including recent work in a mental health setting
Experience in care coordination, telehealth, or supporting individuals with complex mental health needs
Excellent telephone communication and clinical documentation skills
Tech-savvy and confident using digital platforms and electronic health records
A passion for improving mental health outcomes and supporting recovery-focused care
Ability to work independently and manage time effectively
Desirable:
* Experience in injury management, workers' compensation, or motor vehicle accident recovery
* Experience in community mental health, psychosocial support, or rehabilitation/return-to-work coordination
* Experience with chronic disease management, especially when co-occurring with mental health conditions
* Knowledge of trauma-informed care, risk assessment, and multidisciplinary care planning
Work Arrangements
Full-time or four days per week (Mon-Fri, 9 AM – 5 PM AEST/AWST).
Virtual role – You must have a reliable home internet connection and a private dedicated workspace.
