About the Care Support Role

The Care Support Specialist plays a vital role in guiding individuals through their care journey, ensuring they receive high-quality services tailored to their unique needs and goals.

This specialist works closely with clients to identify their individual objectives and collaborates with their service team to consistently deliver high-quality care that aligns with these objectives.

Key responsibilities of this role include:

* Providing information, support, and referrals and completing appropriate intake processes and assessments;
* Developing personalized care plans with the client, which address the client's unique care needs, and outline how services will support them achieve their goals;
* Ongoing monitoring of clients' progress against the identified care goals, conducting reviews and assessments needed to ensure full support;
* Collaborating with service coordinators to secure appropriate service bookings that align with clients' identified goals;
* Ensuring clients can exercise choice and control within the framework and structure of their funded programs.

The Care Support Specialist ensures that all services are provided in accordance with the standard of care as per the relevant funding body as articulated in the relevant program manual.

This role requires a high level of verbal and written communication skills, enabling clear and effective interaction with clients, families, and stakeholders, including those with limited capacity.

We require candidates with a minimum of two years' demonstrated experience in care management, working within the Disability, Aged Care or broader Community Care sectors.

Additionally, we require formal training in Case Management through a recognized course/program, and a current Driver's license (QLD).

Core competencies required for this role include:

* A high level of accuracy in managing client records, contracts, and financial documentation;
* A sound level of problem-solving and decision-making to identify client needs and make informed decisions in line with ethical standards and corporate policies and procedures;
* A sound level of practical application of the cultural, religious, and individual differences of clients and the families/supporters;
* A sound level of competency in developing and implementing sales strategies to attract new clients and expand service offerings;
* Demonstrated ability to provide exceptional client experience through responsive service and continuous improvement.
