Role Description
This is a Part time hybrid role for a Clinical Case Manager based in Bathurst, NSW, with flexibility for some work-from-home arrangements. The Clinical Case Manager will oversee a caseload of clients, providing individualized case management services. Responsibilities include coordinating care plans, conducting assessments, supporting mental health and social services needs, and working closely with clients and their families to optimize care outcomes. The role also involves collaborating with multidisciplinary teams and community service providers to ensure seamless and holistic care delivery.
Qualifications
* Proficiency in Social Work and a strong understanding of social support systems.
* Experience in Case Management and delivering Case Management Services, including maintaining care plans and documentation.
* Knowledge of aged care support and services, with the ability to address various xommunity based health needs.
* Understanding of Social Services and the ability to coordinate resources and support for clients effectively.
* Strong communication, problem-solving, and organizational skills.
* A relevant degree or certification in social work, health, or related fields.
* Experience in the aged care or NDIS sectors is advantageous.
* Demonstrated ability to work independently and in collaboration with multidisciplinary teams.
