We provide a wide range of services across the country to support women and children impacted by family and domestic violence. These services include refuge, children's and parenting programs, counselling, support, men's programs, accommodation, and advice.
Job Description: Specialist Family Violence Case Practitioner
This is a permanent, part-time position offering 30.4 hours per week. You will be working at our Karratha location and reporting to the Team Leader. This role requires the ability to work on a rotating roster and may occasionally require working outside normal business hours.
Your Key Responsibilities
* Maintain effective relationships with clients to encourage self-determination and participation.
* Provide case management support to clients in accordance with TSA FV Model of Care and Case Management model.
* Undertake ongoing risk assessment and safety planning.
* Deliver case management to address individual needs and circumstances from initial intake to exit.
* Develop individual case plans with clients, including a range of interventions to address their needs or goals.
* Assist clients in accessing support services and establishing community supports, and where needed advocate with service providers.
* Monitor and review progress and outcomes of individual case plans, taking action as needed to ensure the plan continues to meet client needs or goals.
What We're Looking For
* A tertiary qualification in Social Work or related field.
* Experience working in the family violence sector is desirable.
* Experience in specialist family violence and/or child wellbeing work is highly desirable.
* Demonstrated case management experience and skills.
* Highly developed time management and work prioritisation skills, with an ability to work autonomously with minimal supervision required.
* Advanced computer skills, including experience in data and record management.
