Description
The Hospital Admission Risk Program (HARP) provides integrated and coordinated care to clients with a chronic disease, aged and/or complex needs who frequently use hospitals or are at risk of hospitalisation.
This position is an integral part of the multi-disciplinary HARP team at Bendigo Health. The main focus of the role is providing high quality social work and care coordination services to clients who access the HARP program, and relevant support to carer and family members in relation to the client’s wellbeing and access to services.
The successful applicant will be subject to the satisfactory completion of Credentialing and Scope of Practice at Bendigo Health.
