Geriatrician Required for Primary Care Outreach
We seek a skilled geriatrician to provide specialist advice on older patient management.
* Offer telephone, videoconferencing, and in-person advice or review of community-dwelling older people with a goal to prevent functional or medical decline.
* Reduce decline in function or hospitalization for frail older people with chronic or subacute frailty or comorbidity.
About the Role
The Geriatrician will collaborate closely with primary care providers to deliver advice, education, and support to community-dwelling older people in the Lower North Shore region.
Primary Responsibilities:
* Provide expert guidance to General Practitioners and other community providers on managing older patients.
* Develop and implement strategies to reduce functional or medical decline among frail older adults.
Required Skills and Qualifications:
* Highly skilled geriatrician with expertise in managing complex cases.
* Excellent communication and interpersonal skills.
