Family Support Practitioner

About the Role:
We are seeking a highly skilled Family Support Practitioner to join our team. As a Family Support Practitioner, you will be responsible for delivering intensive, tailored family support to children, young people, and their caregivers.
You will implement evidence-based interventions, including parenting strategies and life skills coaching. Additionally, you will connect families to community, education, and employment resources.
You will conduct home visits and deliver early intervention strategies to build safety and wellbeing. You will also work collaboratively with Child Safety, government, and community partners to provide wrap-around support.
Maintaining accurate documentation and case notes is essential in line with standards and requirements.

Requirements:
* A valid Working with Children Blue Card and NDIS Worker Screening Check (or willingness to obtain)
* Flexibility to work across all shift types (mornings, evenings, and sleepovers)
* A current driver's licence
* A qualification or progress toward a Certificate IV, Diploma, or Bachelor's in Community Services, Youth Work, Social Work, or a related field
* Experience supporting children, young people, and families, ideally within Family Support, Child Safety, or similar roles
