Learn more - Visit Hamilton About the role Western District Health Service in partnership with Deakin University are undertaking a project to implement Heart Failure guidelines in our community. The successful applicant will join a dedicated team of health professionals working on a project called I-HEART. The project is aimed at reducing hospitalisations for patients diagnosed with heart failure. This is an exciting opportunity to coordinate the project over a two-year period with support both locally and regionally. The project coordinator will have excellent communication skills to engage clients and service providers through the continuum of care. This will require a combination of face-to-face assessment and telehealth consultation with Ischaemic Heart Failure specialists. You will bring: Essential Registered Nurse with current AHPRA registration. Experience as a project officer or research assistant/fellow with a clinical background. Previous/current clinical experience. Desirable but not essential Knowledge or experience in research Previous/current clinical experience in working with heart failure patients For more details take a look at our Position Description.
