About This Role
This is an exciting opportunity to deliver high-quality, evidence‐based clinical services as part of the Adult Community Mental Health Team within the Bayside Division of Addiction and Mental Health Services.
* Provide comprehensive mental health assessments, therapeutic interventions, patient education and discharge planning within an interprofessional team.
* Manage a defined caseload, applying increasingly independent clinical judgement while maintaining accurate documentation and service reporting.
* Collaborate with consumers, carers, families and external stakeholders to deliver person‐centred, recovery‐oriented care.
* Contribute to clinical education, supervision of students and support staff, and participate in quality improvement and research initiatives.
About You
We are seeking a motivated and compassionate Social Worker who is committed to delivering safe, high‐quality mental health care in a community setting.
* Demonstrated ability to provide evidence‐based, patient‐centred mental health assessment and intervention within a multidisciplinary team.
* Strong communication and interpersonal skills, with the ability to build collaborative relationships and engage consumers effectively.
* Proven capacity to manage workload priorities, maintain clinical documentation and contribute to continuous professional development.
* Ability or willingness to participate in clinical supervision, student education, and service improvement activities.
