Task & Responsibilities
* Conduct comprehensive client assessments to identify support needs, goals, and preferences related to health, social participation, housing, independence, and personal development.
* Facilitate onboarding of new clients, ensuring they are informed of available services, understand their rights and responsibilities, nature and extent of their concerns and feel supported throughout the initial engagement process.
* Support clients in setting meaningful and achievable goals, taking into account their individual circumstances, strengths, and aspirations.
* Develop and implement personalised care and support plans in collaboration with clients, their families, and relevant professionals, ensuring alignment with identified needs and goals.
* Meet with clients to understand the nature and depth of challenges they face, and assist in navigating available supports.
* Maintain accurate case notes and documentation, track outcomes, and produce reports to support evaluation and continuous improvement.
* Refer clients to appropriate external services such as health, legal, housing, mental health, drug and alcohol, or employment supports when additional help is required.
* Participate in and lead community consultation and engagement activities to understand emerging needs, identify service gaps, and contribute to the co-design of programs and initiatives.
* Stay informed of relevant legislation, funding programs, and best practices to ensure quality service delivery and compliance with sector standards.
* Contribute to service development and organisational improvement initiatives, bringing insights from frontline experience.
Qualification & Experience
Minimum Requirements
* Qualifications in Community Services, Human Services, Health Sector Management, Aged Care and/or Clinical background (Minimum Diploma)
* Minimum of 3 years’ experience in the industry
Preferred Qualifications
* Valid driver’s licence and willingness to travel within the region
* Blue Card and NDIS Worker Screening Check
* Current First Aid certificate
