As the demand for services grows, Convenient Support Services is seeking an experienced and passionate Disability Support Worker to join their team in West Perth.
The Role
The Disability Support Worker provides person-centred care and support to clients, assisting them to achieve independence, maintain dignity, and participate actively in their communities. The role requires compassion, patience, and the ability to follow complex support plans, while maintaining compliance with NDIS and organisational standards.
This is a full time role with an annual salary set at $80,000 plus superannuation.
Responsibilities
- Provide assistance with daily tasks such as dressing, grooming, and eating, ensuring dignity and respect for each client.
- Support clients in engaging with their community, participating in social events, and accessing local services to enhance their social and life skills.
- Help plan and accompany clients to appointments and recreational activities, ensuring they are safe and supported while promoting independence.
- Assist with medication management and appointments with healthcare professionals, and monitor health as directed by healthcare providers.
- Implement strategies and support plans for managing behaviour in line with individual care plans and professional recommendations.
- Maintain accurate and detailed records of client progress, daily activities, and health information.
- Advocate for clients' needs and rights within the community and ensure they receive the services and support they are entitled to under the NDIS.
- Work closely with a multidisciplinary team to provide insights and updates on client progress, adapting support strategies as needed.
Requirements
In order to be shortlisted for this position, the right candidate needs to have:
- Certificate III in Individual Support (Disability) or equivalent qualification, with comprehensive understanding of NDIS practice standards, safeguarding principles, and client rights legislation.
- Valid First Aid and CPR certification.
- Current National Police Check (within 3 years) and NDIS Worker Screening clearance.
- Minimum of 3 years' experience providing direct support to people with disabilities, including clients with complex physical, behavioural, or psychosocial needs.
- Minimum 3 years of experience providing direct support to individuals with physical, intellectual, or psychosocial disabilities
- At least 3 years of experience assisting clients with personal care, daily living activities, and community participation
- Minimum 3 years of experience implementing behaviour support strategies and following individualised care plans
- At least 3 years of experience supporting clients to build independence and achieve personal goals in line with NDIS plans
- Proven capacity to support clients' independence through skill-building, daily living assistance, and community engagement activities.
- Strong written and verbal communication skills, with the ability to produce detailed case notes, incident reports, and progress documentation suitable for audits.
- Experience collaborating with multidisciplinary teams, allied health professionals, and families to achieve holistic care outcomes.
- Physical capability to safely support clients with mobility aids, transfers, and personal care tasks while adhering to workplace health and safety standards.
- Ability to remain calm and professional in high-pressure or crisis situations, demonstrating resilience and problem-solving skills.
