Job Description
We are seeking a dedicated and compassionate professional to join our team as a Care Manager. As a Care Manager, you will play a critical role in helping individuals with intellectual and/or developmental disabilities achieve their long-term and short-term goals by developing, implementing, and monitoring person-centered Life Plans.
Our ideal candidate will possess excellent communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills. They will be able to advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
Key Responsibilities:
* Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
* Schedule, lead, and actively collaborate with our members and their interdisciplinary team to conduct meetings and assessments ensuring the development of a comprehensive, person-centered Life Plan that reflects the person's needs and desired life goals.
* Implement, update, and monitor Life Plans and facilitate individualized Life Plan reviews and approval processes.
* Ensure integration of all needed and preferred supports and services.
* Maintain ongoing contact with the critical people in a member's life.
* Ensure timely submission of all documentation (Life Plan, Progress notes, etc.).
* Report abuse or neglect immediately when observed or reported.
