We have an exciting opportunity for you with our Single Aged Care Assessment team. Our goal is to provide a cohesive and efficient approach to caring for the elderly ensuring comprehensive support for our clients.
About The Role
Centacare is looking for Non-clinical Assessors based in Hervey Bay who are committed to enhancing quality of life for elderly individuals.
The primary purpose of the Aged Care Assessor is to evaluate the needs of elderly clients to determine appropriate care and support services.
Responsibilities
* Conduct home support assessments using the Integrated Assessment Tool (IAT).
* Collaborate with clinical assessors to address clinical questions during assessments.
* Collaborate with healthcare providers to ensure clients receive client centred care within Government frameworks and applicable funding criteria.
* Develop Support Plans that reflect the client's goals and needs.
* Record assessment information accurately and comprehensively.
Essential requirements
* Minimum one year case management experience in assessment and goal orientated support planning in aged, community, health preferred
* Previous knowledge or work experience in the aged care system preferred
* Unrestricted Australian Drivers Licence.
* National Police Check.
* NDIS Blue Care and Yellow Card.
* First Aid and CPR.
* Proof of right to work - Australian Birth Certificate, Passport, citizenship or Visa.
