We are seeking an energetic and driven Community Engagement Consultant to join our Central Coast team. This is an individual contributor role focused on referral management, new client acquisition and onboarding to support the continued expansion of our services.


Position Overview

As a Community Engagement Consultant, you will be responsible for building strong relationships within the community, managing new lead consultation meetings, new client sign‑ups and facilitating a seamless onboarding process. You will engage with key stakeholders such as hospitals, GPs, pharmacies, and community organisations to strengthen referral pathways and position Home Care Assistance as a trusted provider of aged and disability care services .

You will work closely with our Care Partners and Operations Teams to ensure a smooth client journey from enquiry to ongoing care.


Key Responsibilities

* Manage client enquiries from initial consultation to onboarding, ensuring a smooth transition into services.
* Meet with prospective clients and families to discuss home care options, funding eligibility, and Home Care Assistance services.
* Gather key client information and develop the foundation of their care plan before transitioning to the Case Manager.
* Build and maintain referral relationships with hospitals, GPs, pharmacies, brokers, and community organisations to drive client acquisition.
* Attend industry and networking events to strengthen partnerships and establish Home Care Assistance as a trusted provider.
* Maintain accurate client records in HubSpot CRM, track lead generation efforts, and ensure timely completion of onboarding documentation.
* Conduct timely and strategic follow‑up calls with prospects, maintaining a touchpoint strategy.
* Collaborate with internal teams, including Care Managers, Care Worker Managers and Rostering Teams, to ensure successful client integration.
* Stay informed on aged care reforms and industry regulations, ensuring compliance with policies and ethical standards.
* Provide insights and feedback to internal teams to support continuous improvement and enhance client and stakeholder engagement.


About You

* You thrive on relationship management both face‑to‑face and over the phone and can work autonomously.
* You have hospitality experience that is transferable to the age care sector.
* You are a self‑starter and have the will to learn a new sector.

* Customer Service Focused – Dedicated to providing exceptional service across all communication channels (phone, email, and in‑person).
* Strong Communication Skills – Ability to adapt communication style to different audiences and clearly explain home care services.
* Attention to Detail – Excellent written and verbal skills to ensure accurate documentation and client interactions.
* Teamwork & Collaboration – Willingness to work closely with multiple departments and support team members as needed.
