This role focuses on creating and implementing case plans that achieve permanency goals. It involves working closely with children, young people, their families, and carers to ensure every voice is heard and every need is met.
The Family Based Care team supports and monitors placements, guiding carers in foster care journeys and helping them implement effective strategies. As a Case Manager, you will build strong relationships with the children and their families.
