Imagine Rural - Southern NSW Local Health DistrictWhat You'll Be DoingThe Care Navigator provides short-term care coordination for complex presentations of older consumers and those with chronic disease in both the acute and community settings. The position, based in community health, will work as part of the broader acute, community and primary care teams to plan and navigate the post-discharge options of care with the consumer/ carerSelection CriteriaCurrent Registration with the Australian Health Practitioners Regulation Agency (AHPRA) (Nursing, Occupational Therapist, Physiotherapist, Psychologist) or eligibility for membership with the peak professional association (Social Workers, Dietitian, Speech Pathologist).For Nurses post-graduate tertiary qualifications in the relevant field with at least 5 years’ post registration experience OR 5 years post registration experience plus 2 years working with frail older consumers and those with chronic illness; For Allied Heath clinicians demonstrated extensive specialist knowledge of frail older consumers and those with chronic illness. For Pscyhologists, as above for Allied Health and outline qualifications and experience in line with the Award.Demonstrated knowledge and understanding in applying research and evidence based practice to the workplace with the ability to think critically, problem solve and link outcomes to the principles of continuous quality improvement in the clinical speciality field.Demonstrated high level of interpersonal and communication skills, including written, verbal and computer skills and an ability to work collaboratively in a multidisciplinary team.Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.Demonstrated ability to lead family/ case conferences, discharge planning and coordinate transfer of care for complex presentations across the continuum of care. This includes a demonstrated knowledge of the eligibility criteria and referral process for post discharge options of care.Current unrestricted drivers’ licence and the ability to maintain. Ability and willingness to drive/travel as the role requires.
