Summary
Our client Luminous Care is seeking to add a Social Worker to its committed, participant‑focused team in South Australia. An experienced registered NDIS provider, our client delivers in‑home care, community participation, nursing and supported independent living services.
The role will further enhance participants' independence, wellbeing and social inclusion through holistic assessment, advocacy and tailored support planning.
Responsibilities
* Conduct psychosocial and functional assessments for new and existing participants.
* Identify risks, support needs and barriers, and prepare tailored support plans.
* Review participant plans regularly to ensure they align with changing needs.
* Provide case management for participants with complex psychosocial or behavioural challenges.
* Offer counselling, crisis support and short‑term therapeutic interventions when appropriate.
* Work with families, carers and internal teams to stabilise participant circumstances.
* Liaise with hospitals, GPs, mental health services and community organisations to coordinate care.
* Coordinate referrals to allied health, housing providers, advocacy bodies and emergency services.
* Support participants to access and navigate community supports that enhance independence.
* Conduct risk assessments and implement safeguarding strategies in line with NDIS requirements.
* Assist with incident reviews, behaviour support planning and reducing restrictive practices.
* Maintain accurate case notes, assessments and reports in compliance with NDIS standards.
* Advocate for participant rights and inclusion across health and social service systems.
* Support participants to make informed decisions about their care and goals.
* Build long‑term relationships that encourage participant confidence and community participation.
* Work closely with coordinators, team leaders and support workers on complex cases.
* Provide guidance to staff on trauma‑informed and person‑centred support approaches.
* Participate in internal reviews, quality improvement activities and training sessions.
Qualifications
* Bachelor's degree in Social Work, with eligibility for professional membership or registration where required in Australia.
* Minimum one year's post‑qualification experience providing social work or case management services within disability or community settings.
* Strong understanding of NDIS principles, participant pathways and person‑centred planning within community‑based support environments, applied effectively.
* Demonstrated ability to manage a diverse caseload, prioritise competing demands and meet organisational and regulatory timeframes.
* Highly developed communication, counselling and advocacy skills, with confidence engaging participants, families and multidisciplinary stakeholders.
* Proven ability to build trusting relationships with people from diverse cultural backgrounds, respecting choice, dignity and independence.
* Accurate report‑writing and record‑keeping skills, with competence using client management systems and common office software.
* Current driver's licence, reliable transport and willingness to travel between participant homes, community locations and office sites.
