**About the Opportunity**
- Provision of special work services to clients with Neurodegenerative Diseases, such as Motor Neurone Disease
- Work closely with families and carers
- Work in conjunction with other services such as NDIS providers
- Home visiting and some clinic work
- Work with MDT
- Work from point of clients diagnosis to end of life
- Community experience desirable
**What You'll be Doing**
This position is responsible for providing specialist Social Work service to consumers and their carers particularly in the Neurodegenerative Disease Supportive Care Service to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

**How to Apply**
- A Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
- Extensive post graduate experience working as a Social Worker in relevant clinical areas in a health setting, including a demonstrated understanding of complex health conditions and the impact on wellbeing.
- Demonstrated ability to lead complex casework including communication and collaboration with the patient and other stakeholders in order to maximize positive patient outcomes and collaborative partnerships
- Demonstrated counselling skills to address client-distress, and ability to provide counselling / therapies that focus on the psychological, behaviour, relational or social causes of distress
- Experience working independently and collaboratively within a multi-disciplinary team.
- Demonstrated high level communication skills (written and verbal), computer and prioritisation skills
- Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and the clinical specialty.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
