Job Description
The Ashburton Drive Primary School is a part of the Connected Community School trial, a four-year initiative aimed at connecting students, families and the school community with services and supports to improve education and wellbeing outcomes.
The school is committed to actively involving the wider community to support student and staff wellbeing and working together to implement whole-school practices.
The successful applicant will play a vital role in supporting a whole-school approach that strengthens partnerships, builds positive community relationships and supports students and their families to improve student wellbeing, learning and life outcomes.
They will build rapport with the school community, undertake assessments of referred students, identify suitable services and supports, prepare plans and engage in social work intervention with students and families including interviewing family members and making home visits.
Maintaining accurate records, case notes, data and reports will support evidence-informed practice and contribute to trial evaluation. This is a unique opportunity to support established school programs and partnership initiatives and assist with the implementation of a locally led, culturally safe and inclusive whole-school approach.
