We are currently seeking a dedicated Practice Support Worker to join our Foster & Kinship team located in the Toowoomba region on a permanent part-time basis (4 days per week).
A Practice Support Worker works collaboratively with the case management team to support and maximise the effectiveness of support plans, ensuring planned and permanent goals for children or young people (CYP) are achieved.
The role involves working with CYP with a range of developmental, behavioural, and psychological needs and their families, carers, and support teams, providing support through coaching, mentoring, training, and modelling; along with facilitating and coordinating available supports.
The Practice Support Worker will:
1. Actively support the implementation of the plans by providing direct support to the child or young person and their families and/or carers.
2. Undertake direct support functions as required and under authority to facilitate plan goals, including supporting contact, transportation, behavioural observation and recording, in-home and in-community support to CYP, carers, and permanent plan stakeholders.
3. Actively support the implementation of the plans by facilitating and coordinating supports available in the community.
4. Support the implementation of sympathetic and effective client feedback and monitoring systems, providing support for their effective use and review.
5. Provide training, coaching, mentoring, and other support for the implementation of strategies and plans to bring about behavioural change in children, parents, and carers.
6. Contribute to monitoring and reviewing assessments and interventions for effectiveness and validity.
7. Assist in event planning and coordination.
8. Support the Foster and Equipping Teams with tasks to ensure adequate service delivery.
9. Complete and regularly update session plans, outlining support provided to CYP and households.
10. Complete case notes outlining observations and supports implemented.
About you:
To be successful, you should demonstrate knowledge and experience relevant to working with young people, residential and/or youth programs, along with:
* A relevant qualification and/or Diploma in Human Services, community development, or related fields (desirable but not essential).
* Experience developing positive rapport with stakeholders, including community members, young people, carers, and staff.
* Experience working with indigenous communities and culturally diverse groups.
* Understanding or experience in child protection or similar industry.
* Knowledge of complex trauma, neglect, and reasons children and young people are placed in out-of-home care.
