About The Role
This is an exciting opportunity for a Registered Nurse passionate about Aged Care to join the Nursing Team at Bathurst Health Service. In this role, you will work as an integral member of the multidisciplinary team responsible for coordinating care for older persons who present to the Emergency Department. This involves undertaking assessments, initiating referrals to other Health Professionals and providers, and maintaining a patient focused model of care, with the aim of achieving the best and most appropriate health care outcomes of older persons. You will be supported to achieve clinical excellence, encouraged to learn, and to build strong relationships with other Health Care Professionals.
Responsibilities
* Coordinate care for older persons presenting to the Emergency Department.
* Undertake assessments and initiate referrals to other health professionals and providers.
* Maintain a patient-focused model of care to achieve the best health care outcomes for older persons.

* Employment Type: Permanent Full Time
* Position Classification: Registered Nurse
* Remuneration: $72,152.53 - $101,299.48 per annum (+ superannuation + salary packaging options)
* Location: Bathurst, NSW
* Hours Per Week: 38
