Job Description:
Provide compassionate and high-quality end-of-life care to clients and their families in home and community settings. Work collaboratively with palliative care and wider community service teams to ensure coordinated, person-centred care.
Key Responsibilities:
* Deliver comprehensive end-of-life care to clients, addressing their physical, emotional, and spiritual needs.
* Collaborate with healthcare professionals to develop and implement individualised care plans.
* Support clients and their families through the dying process, promoting dignity and respect.
* Contribute to a supportive team environment, fostering open communication and mutual respect.
Required Skills and Qualifications:
* Current Registered Nurse licence and registration certificate in the employing state.
* Bachelor of Nursing or an equivalent qualification.
* Mid to senior level proficiency in general nursing with clinical expertise and a person-centred care approach.
* Prior experience or ongoing pursuit of qualifications in palliative care, pain management, or acute care is highly valued.
* Demonstrated aptitude in effective communication and collaboration with clients and stakeholders to address care requirements.
