Position Summary

The Aged Care Coordinator is responsible for managing the day-to-day operations of Purple House's remote aged care services, including the Yuendumu Old Peoples Program (YOPP) and Mt Liebig Aged Care. The role involves coordinating client services, leading the team, and ensuring a high standard of care and support for all clients.

Key duties include managing client care, supervising staff, maintaining workplace health and safety, and ensuring compliance with relevant standards and regulations. The Aged Care Coordinator will also focus on improving service quality and implementing best practices to enhance the aged care program.

* New appointments are probationary for the first six months.
About Purple House

Our mission is to improve the lives of people with renal failure, reunite families and reduce the impact of kidney disease in our communities. We are run by Aboriginal people for Aboriginal people and work to provide culturally appropriate dialysis services in remote communities, helping people to get home to country and family.

Standard Requirements
* Demonstrate a courteous and respectful manner and communicate effectively with internal and external stakeholders.
* Demonstrate cultural awareness and knowledge of cultural safety and Indigenous community control.
* Maintain confidentiality of information about clients, other staff members or community members.
* Work to achieve standards of best practice and promote continuous quality improvement.
* Demonstrate safe work practices and promote a safe work environment.
Functions
* Positively promote Yuendumu Old Peoples Program (YOPP) / Mt Liebig Aged Care as a Purple House service.
* Case management, care coordination, and person-centred care that supports clients' rights, dignity, and independence.
* Liaise with, and refer to, complementary service providers to provide services for YOPP/Mt Liebig clients (e.g., Allied Health, Clinic, Memory Service).
* Assist clients with Activities of Daily Living such as personal care, toileting, and mobility.
* Provide individual support, transport, and laundry services.
* Demonstrate professional administration skills, ensuring compliance with consumer-directed care requirements and Aged Care Quality Standards.
* Follow safe food handling practices and assist with meal preparation and delivery.
* Ensure timely and relevant kitchen stock ordering.
* Ensure all services are provided in accordance with relevant policies, procedures, and regulations.
* Liaise with the Malpa Manager, Aged Care Manager Remote, and Finance Team to develop and monitor the remote aged care budget.
* Maintain accurate record-keeping and submit reports in a timely manner.
* Provide operational support and auditing of assets.
* Staff management, including rostering, guidance, and performance management, to ensure staff follow organisational policies and procedures and that client needs are met according to individual care plans.
* Provide mentoring and training to aged care staff.
* Work collaboratively and maintain effective relationships with internal and external stakeholders.
* Promote a safe work environment in accordance with legislative requirements, adhere to Work Health and Safety guidelines, and report all incidents and near misses.
* Other duties as required within trained capacity and level.

* You have experience working for an Aboriginal organisation and awareness of the issues pertaining to this region.
* You have a Certificate III Aged Care (minimum) or equivalent work experience (Community Services, Disability work)
* You have a Food Safety Handling supervisor certificate.
* You have experience leading and mentoring a team.
* You have general knowledge of community aged care and key standards for service delivery and case management.
* You have knowledge of nutritional requirements and experience in food preparation and planning.
Key Competencies (Selection Criteria)

Essential

* Relevant qualification in Aged Care, Disability, or Health (Diploma or higher).
* Proven experience in the delivery of Aged and/or Disability Care Services, including associated programs, industry standards, and reforms.
* Strong knowledge of the Aged Care Quality Standards and principles of Client and Consumer Directed Care.
* Demonstrated proficiency in computer skills, including Microsoft Office (particularly Word, Excel, and Outlook), and experience with client data management. Ability to quickly learn new software.
* Understanding of, and experience with, cultural protocols, with a demonstrated willingness to work respectfully with Indigenous communities and cultures.
* 3-4 years of experience in a related management or leadership role.
* Current Senior First Aid and CPR Certificate.
* Food Handling Safety Supervisor Certificate.
* Valid NT Driver's License (C Class minimum).
