About the Role
The Clinical Care Coordinator is responsible for leading both the clinical and care teams across the home, in consultation with the Home Manager, to ensure the needs of each resident are met and the team understands residents' identities, cultures, beliefs and life experiences. The role supports clinical governance and may undertake a specialist clinical portfolio (e.g., education, palliative care, infection control) to meet the home's needs. This position is part of an integrated team working within an established framework to help residents live their best life.
Key Responsibilities
* Ensure clinical and any specialised nursing care needs of residents are met and documented in an individual resident care plan by clinical staff
* Maintain resident and family involvement in decision making, considering needs, choices, independence, dignity and identified risks in the care plan
* Oversee and implement all scheduled mandatory and voluntary ACFI reappraisals as required
* Hold RNs and ENs accountable for daily case reporting, care planning and addressing patient concerns
* Ensure regular clinical checks are conducted and gaps addressed in a timely manner
About You / Qualifications
* Current registration as a Registered Nurse with AHPRA
* Minimum of 3 years in a management role within the aged care sector with a proven track record
* Knowledge of the aged care sector, ability to identify changing trends and ensure best practice
* Understanding of and ability to manage human resource issues with effective negotiation skills
* Effective listening and communication skills (oral and written)
* Ability to use information technology (e.g., Word, Excel) and to learn new skills as required
* Working knowledge of WH&S and Infection Control
