Clinical Care Coordinator Role
We are seeking a highly motivated and enthusiastic registered Nurse to work as a Clinical Care Coordinator, providing clinical leadership across all aspects of the care operations. This role involves working collaboratively with management, participants, families, guardians, disability support workers, and community agencies to facilitate high-quality, effective, and participant-focused care.

As a registered Nurse, you will have the opportunity to maintain your registration and pursue further professional development without hospital pressure. You will work closely with the team to provide exceptional care and support to our participants.
Key Responsibilities
* Provide clinical leadership and guidance to care teams
* Collaborate with internal and external stakeholders to achieve quality outcomes
* Develop and implement care plans that meet individual needs
* Maintain accurate records and reports
* Participate in professional development opportunities to enhance skills and knowledge
Requirements
* Degree in nursing or allied health and significant experience in a similar role
* Current registration with the appropriate professional body (AHPRA etc)
* Extensive understanding of the National Disability Insurance Scheme (NDIS) and current legislative requirements
* Well-developed clinical judgement skills with the ability to apply critical thinking and problem-solving skills
* Highly developed interpersonal and communication skills
* Ability to implement change and improvements in care practices
