Long Service Leave Cover – ASAP until 30/10/2022. Multiple Locations – South‐East Melbourne.

About You
* AHPRA registration (or eligibility)
* Skills/knowledge in functional assessments, home mods, AT/equipment
* Strong communication, organisation and problem‐solving skills
* Ability to work with diverse and CALD communities
* Commitment to continuous learning and quality care
Your Day‐to‐Day
* Conduct OT assessments and develop client‐centred care plans
* Complete home visits and recommend equipment/home modification
* Submit applications to My Aged Care, CHSP, HACC PYP, Support at Home, SWEP and Geat2go
* Provide coaching and self‐management education to clients and carers
* Work closely with Physios, Nurses, Podiatrists, AHAs and more
* Participate in supervision, case reviews and quality improvements
* Maintain accurate documentation and meet service KPI's
