About your new role
You will be supporting clients in various locations and play a vital role in providing high-quality care to clients living at home. Your key responsibilities will be:
* Delivering Clinical Care
* Conducting Comprehensive Assessments and regular reviews
* Care Planning and Coordination
* Advocating for Clients
* Providing Health Education
* Monitoring Health and Reporting Outcomes
* Networking and Promoting BaptistCare Services
Your working hours
This position is full time, 38 hours per week. You can work from home with flexible arrangements and have access to nationwide health and fitness facilities. Travel will be reimbursed as per the BaptistCare NSW and ACT Aged Care Enterprise Agreement.

Requirements
* Registered Nurse qualifications with current registration
* 12 months clinical experience in acute or aged care sector
* Current unrestricted driver's licence with minimum Third-Party Damage Insurance
* Excellent communication skills
* Intermediate computer skills
* Sound decision making and problem-solving skills
* Able to work independently and as part of a team
* Demonstrate an understanding and appreciation of BaptistCare's Christian identity
