About the Opportunity The Health Care connections worker is part of a small passionate team providing high quality assertive outreach support to people experiencing homelessness. The main aim of this role is to connect people who are homeless with relevant local primary health care providers that meet their needs. The role involves empowering people to address barriers prevent them from accessing health services. The role builds and maintains relationships with local primary health care providers and other services.
