Job Title: Disability Support Coordinator
We are seeking a dedicated and passionate individual to join our team as a Disability Support Coordinator.
About the Role:
In this dynamic role, you will be responsible for assisting people with disabilities, their families, and carers to build and pursue their goals to live their best life, exercise choice and control, and engage with the National Disability Insurance Scheme (NDIS).
* You will assist people with permanent and significant disabilities to apply for NDIS supports, develop goals, and connect them to support within the local community.
* Collaborate with participants to develop effective plans that meet their needs and aspirations.
* Support participants to navigate and optimise their engagement with the NDIS.
* Enhance community knowledge of the NDIS and create new opportunities for participants to access services and support.
Responsibilities:
* Build and maintain relationships with people with disabilities, their families, and carers.
* Demonstrate relevant experience and/or lived experience in disability support.
* Develop and implement effective plans to achieve collaborative outcomes.
* Foster strong connections between people with disabilities and local community resources.
* Meet identified contractual Key Performance Indicators (KPIs).
Requirements:
* You must be an approachable, positive, passionate, and respectful person who can build and nurture relationships easily and confidently.
* Demonstrated relevant experience and/or lived experience is essential.
* Excellent communication and interpersonal skills are required.
* A deep understanding of the local community and available options for social and economic participation is necessary.
* You must be proficient in Microsoft Office 365 and have basic computer skills.
* A Working with Children and/or Vulnerable People Check is required.
