We're looking for an experienced Team Lead – Care Coordination to guide a passionate team, while rolling up your sleeves and staying connected to clients yourself.

About The Role
As Team Lead, you'll provide clinical leadership and day‐to‐day support to our Care Coordination team, while managing your own care coordination caseload. You'll help ensure services are high‐quality, client‐centred, and integrated with general practice and community partners.
This is a hands‐on leadership role where your expertise helps people navigate the health system, build health literacy, and stay well — and out of hospital.
What You'll Be Doing
Lead, mentor and support a team of Care Coordinators delivering complex, integrated careProvide clinical guidance on participant assessment, care planning and pathwaysOversee referrals, intake, waitlists and service flowEnsure services meet organisational policies, funding requirements and quality standardsCollaborate with senior leaders on reporting, audits and continuous improvementBuild strong partnerships with GPs, health services and community organisationsPromote the program across the health sector and communityKeep your clinical skills sharp with your own care coordination caseload
About You
You're an experienced health or human services professional who:
Has relevant tertiary qualifications (e.g. nursing, social work, health, community or human services)Is registered with AHPRA or eligible for AASW membership (as relevant)Brings strong operational and clinical leadership experienceHas strong experience in integrated, person‐centred health careCan confidently assess complex needs and develop meaningful care pathwaysCommunicates clearly, compassionately and inclusivelyThrives in collaboration and relationship‐buildingIs organised, adaptable and comfortable with data, systems and reporting
Bonus points if you have experience in community‐based services or telehealth delivery.
