About us: Gladstone Region Aboriginal and Islander Community Controlled Health Service Limited (“GRAICCHS”) trading as Nhulundu Health Service is an Aboriginal Community Controlled Health Service operating in Gladstone and the greater Central Queensland Region delivering a comprehensive range of primary health care, aged care and community support service functions. GRAICCHS mission is that all Aboriginal and Torres Strait Islander Community members, and the community as a whole, have the right to access equitable and high-quality health care, regardless of culture, ethnicity or where they live. Job Description About the position: This 12‑month maximum term full-time role offers valuable experience within an established health service. As ITC Access and Care Coordination, you will work within our Primary Health Care team to provide practical services and support to the community. This role is nonclinical. Your responsibilities will include: Provide primary and secondary intervention services to improve health outcomes for Aboriginal and Torres Strait Islander clients; Coordinate referrals, access to services, data reporting, and supplementary services including visiting chronic disease specialists and allied health providers; Advocate for clients and assist in removing barriers to healthcare access through liaison with local Indigenous communities and groups; Advocate for clients and assist in removing barriers to healthcare access through liaison with local Indigenous communities and groups; Maintain effective relationships with health service providers and stakeholder networks to increase access; Confirm client eligibility and register clients in ITC program systems; Send program acceptance letters to GPs and welcome letters to clients; Maintain and update referrals, recall systems, and administrative processes to support chronic disease care; Provide recommendations to the Clinical Practice Manager to improve efficiency. Desired Skills and Experience About you: To be successful in this position, we are seeking an individual that is: Passionate about improving health outcomes for Aboriginal & Torres Strait Islander communities; Highly organised with strong communication and interpersonal skills; Able to advocate effectively for clients and navigate health systems with cultural sensitivity; Confident managing referrals, scheduling, and client support pathways; Experience working in, admin, Aboriginal Community Controlled Health or similar environments.
