Required Skills and Qualifications:
* A Certificate III or higher in Case Management, Aged Care, Disability, Community Services, Youth, or a related field
* Previous experience in case management within aged care, disability, or youth services
* Excellent communication and interpersonal skills
* A valid driver's license and access to a reliable vehicle
* Confidence to work autonomously while collaborating with internal and external stakeholders
