We are seeking a passionate Case Manager to join their team and support clients in managing their Home Care Packages (HCP).
About the Role
1. Develop and manage individualized care plans tailored to clients' needs
2. Assist clients in accessing services that promote independence and well-being
3. Regularly review and adjust care plans to align with changing circumstances
4. Advocate for clients to ensure they receive the best possible care
5. Ensure compliance with government regulations and funding guidelines
About You
- Experience in case management, aged care or community services
- Strong understanding of Home Care Packages and aged care legislation
- Excellent communication and problem-solving skills
- A genuine passion for enhancing the lives of older individuals
- Relevant qualifications in social work, aged care, or a related field
- A commitment to delivering values-driven, person-centered care
